What is Mounjaro?

Mounjaro is a once-weekly injection licensed for weight management. The active ingredient is tirzepatide. Unlike single-action medicines it works on two appetite hormone pathways at once: GLP-1 and GIP.

By acting on both it helps steady appetite, slow digestion and improve blood sugar control. That combined effect is why trials report greater weight loss than with single-pathway options.

In a 72 weeks study people on the highest dose lost an average of 20.9% of their body weight when tirzepatide was combined with lifestyle support.

How it works

How Mounjaro works

Clinician-led treatment works with the body's own appetite signals rather than against them, which is why the weight tends to stay off while you are on it.

  • Quieter appetite

You feel less pulled towards food between meals, so choices get easier.

  • Fuller for longer

Your stomach empties more slowly, so smaller meals still satisfy.

  • Fewer cravings

The constant background thinking about food tends to settle down.

  • Steadier blood sugar

Fewer energy dips through the day, which means fewer hunger spikes.

Two pathways

GLP-1 vs GIP

Both hormones act on similar parts of the body, but they do different jobs. Working on two of them at once is the reason the trial figures come out higher.

GLP-1

GIP

Slows stomach emptying, helping you feel fuller for longer

Acts on fat tissue and may alter how the body stores and uses energy

Reduces appetite and the drive to eat

Adds another signal for appetite and energy regulation

Helps reduce food cravings and snacking

May influence how the body handles and stores fat

Helps control the rise in blood sugar after eating

Provides an additional layer of blood-sugar control

Wegovy vs Mounjaro for weight loss

Both are weekly injections and both quiet appetite. The difference is how many gut hormone pathways each one copies — semaglutide copies one, tirzepatide copies two — and that gap shows up in the trial figures.

Using it

How do you use the Mounjaro pen?

Pre-filled pen, four weekly doses, one press a week. Three stages, start to finish.

1

Prepare your pen

Prepare

More detail
  • The pen comes pre-filled, so there is nothing to dial or prime
  • One pen holds four weekly doses
  • Have a sharps bin out on the surface before you start
2

Choose your injection site

Choose a site

More detail
  • Stay at least 5cm clear of your belly button
  • Rotate sites week to week so the skin has time to recover
  • Skip anywhere sore, bruised or hard to the touch
  • Tummy. Easiest to reach. Stay at least 5cm clear of your belly button.
  • Thigh. The front and outer part, halfway between hip and knee.
  • Back of the upper arm. Fine if someone else is doing it for you, awkward on your own.
3

Inject and dispose

Inject and bin

More detail
  • Same day each week, with or without food
  • The whole pen goes straight into the sharps bin, never the household bin
  • If you are not sure the full dose went in, message us rather than repeating it

Keep unopened pens in the fridge between 2 and 8 degrees. Once out, a pen is fine below 30 degrees for up to 30 days. Never freeze it.

Safety

Common side effects for Mounjaro?

Side effects are common when you start any new medicine. Most are mild and ease off as your body settles, and going up more slowly helps more than anything else.

Like all medicines, Mounjaro has side effects. In the SURMOUNT trials, around 18% of people felt sick, 15% had diarrhoea and 12% had constipation. These were usually mild, worst in the first week after a dose increase, and faded within a few days as the body settled.

The most common Mounjaro side effects include

  • Feeling sick
  • Diarrhoea
  • Constipation
  • Being sick
  • Indigestion and reflux
  • Tummy pain
  • Reduced appetite
  • Tiredness

These are usually worst in the first week after a dose increase and settle within a few days. Going up more slowly helps more than anything else.

We will not prescribe it if

  • You are pregnant, breastfeeding or trying to conceive
  • You have had medullary thyroid cancer or MEN 2
  • You have had pancreatitis
  • You are already taking another GLP-1 medicine

Questions

The things people actually ask

Do you put weight back on after stopping Mounjaro?

Most people regain some weight after stopping. In the SURMOUNT-4 trial, participants who switched to placebo after reaching a maintenance dose regained a large share of the weight they had lost, while those who continued treatment carried on losing. Obesity is a long-term condition, so weight tends to return once the medicine stops unless eating patterns, activity and sleep have changed alongside it. Stopping is usually best planned with a prescriber rather than done abruptly.

How long does Mounjaro take to work?

Appetite usually changes within the first one to two weeks, often before any change on the scales. Weight loss typically becomes clear from around the third month, and in the trials it continued steadily for about 72 weeks before levelling off. The rate of loss depends on dose, starting weight, diet and activity.

How much weight can you lose on Mounjaro?

Average weight loss was 20.9% of body weight over 72 weeks on the 15mg dose in the SURMOUNT-1 trial, and 15.0% on 5mg, compared with 2.4% on placebo, all alongside diet and exercise support. Around nine in ten participants lost at least 5% of their starting weight. Individual results vary widely, and no outcome can be guaranteed.

Do you lose muscle on Mounjaro?

Some lean tissue is lost with any substantial weight loss, typically a minority of the total. Loss is reduced by eating enough protein at each meal, generally around 1.2 to 1.6g per kilogram of body weight per day, and by resistance training two or three times a week. Preserving muscle helps maintain strength, mobility and resting energy expenditure.

What is the difference between Mounjaro and Wegovy?

Both are once-weekly injections licensed in the UK for weight management, but they act differently. Mounjaro (tirzepatide) acts on two hormone pathways, GLP-1 and GIP, while Wegovy (semaglutide) acts on GLP-1 alone. Average trial weight loss is higher with tirzepatide, at around 20.9% over 72 weeks against about 14.9% for semaglutide over 68 weeks. The right choice depends on medical history, tolerance and clinical judgement.

Is Mounjaro the same as Ozempic?

No. They contain different active ingredients. Ozempic is semaglutide and acts on one hormone pathway, whereas Mounjaro is tirzepatide and acts on two. Ozempic is licensed in the UK for type 2 diabetes rather than for weight loss, so prescribing it purely for weight management would be outside its licence.
